New clients: Please follow prompts through the button for preregistration to create a new client inquiry, if you have not been in touch with Passages PT before.
Returning clients: ALL prior Passages PT clients are automatically preregistered into the Passages PT portal. Please do not request a new registration. Look for the prompt lower on the screen for “Forgot Password”.
This will allow you to have access later for intake questionnaires and option of convenient payments. If you do not receive an email, please check spam folder.
Medicare and Tricare patients will be sent separate insurance intake forms.
